Your security questions have not been set. If you are unable to log in, please have a user with manager permissions update your password.

This error will occur if a new user tries the Forgot Password option before setting up security questions. The password for the user was created along with the username. If that password is unrecoverable, a user with "Edit User" permissions will need to update the password by:
